Katie Worzel, Owner
Katie Worzel, owner of Cornerstone Care, is a licensed clinical social worker (LCSW) in the state of Missouri. She obtained Bachelor degrees in Social Work and Sociology from Evangel University in Springfield, Missouri, and her Master of Social Work from Washington University in St. Louis. She has many years of experience working with clients who are geriatric, mentally ill, physically disabled, or have been diagnosed with dementia or Alzheimer’s. Her goal is to provide intuitive services, guided by the client’s needs and wishes that will offer peace of mind for the client and family as a whole.
Lori Jurgiel, Care Manager
Lori Jurgiel, Care Manager, is a Certified Care Manager by the National Academy of Certified Care Managers (NACCM) and an Advanced Professional member of the Aging Life Care Association (ALCA formerly known as NAPGCM). She obtained a Bachelor degree in Business Administration from University of Missouri, and her Masters degree in Gerontology from Lindenwood University in St. Charles, Missouri. Lori offers many years of experience working with clients who are geriatric, mentally ill, physically disabled, or have been diagnosed with dementia or Alzheimer’s. Lori’s goal for her families is to develop a personalized Care Plan to allow maximum independence, meaningful life engagement while providing a safe living arrangement for clients.
Getting Started With A Care Manager
Meet With A Care Manager
A care professional will meet with your family to talk about your specific concerns and goals. This meeting can be in our office, in your home or favorite coffee shop, or virtually-whatever works best for your family.
Immediate Needs Met
Immediate needs will be addressed- immediately! Your Care Manager will offer guidance, advocacy and direct referrals to appropriate resources. He or she can then coordinate the initiation and continuation of these service, or simply offer support to the family as you contact referrals independently.
Planning For The Future
As the Care Manager gets to know your family, they can assist you in preparing for the future and developing a family plan that will allow for consistent, supportive services to your loved one. This includes legal and financial planning, family counseling related to long-term care options and end of life planning.
